Sam Milhoan is the adoptive father of FBI agent Elizabeth Keen in “The Blacklist”.
Background[]
In 1990, After Raymond Reddington is killed and Katarina Rostova went on the run from multiple governments, Kathryn Nemec showed on his doorstep asking him to take care of their daughter: Masha. Despite some reluctance, Sam agrees to look after her until her mother comes back, assuming it is temporary.
Katarina calls Sam and he barrages her with questions about about what is going on, but Katarina doesn’t give him an answer. Instead she tells him to put Masha on the phone and Sam angrily tells her to take her off his hands before he hands the phone to Masha at Katarina’s insistence. After she handed the phone back, Sam again asked for answers but Katarina simply said to take care of Masha and hung up the phone.
After that, Sam became Masha’s adoptive father and at some point changed her name to Elizabeth. Sam cared for her as best he could, though she believes she cared for herself. In 1991, Sam walks into Kathryn’s office, telling her “Reddington” wants to see her. Kate angrily refuses to meet the man since he triggered a chain reaction of events that forced her to abandon Masha and Katarina. Sam tells her that Masha, now known as Elizabeth, is in danger and she agrees to meet with him.
Sam was among the very few who knew that Raymond Reddington is not the real Raymond Reddington, but an imposter who took over his identity over 30 years ago.
Season 1[]

Sam calls to tell Lizzie that he is having another test for the cancer. Lizzie tries to go, but gets caught up in work, so Tom goes to see Sam. Red goes to pay his respects, but smothers Sam to death when Sam tries to call Lizzie and tell her a secret about her biological father. Red later states that Sam was very sick and confused otherwise he would never have tried to tell her a secret that was too dangerous and that had not been his secret to share.
“Berlin: Conclusion”[]
Red tells Lizzie that the way Sam told the story was that one night an old friend turned up at his door scared who said he was leaving town and was in danger and that he needed somebody to care for a little girl (Lizzie). The girl's father had died that night in a fire and so Sam took her in and cared for her as his own and sheltered her from the truth about her biological father so that she would not be in great danger. Red tells her that taking Sam's life may have been the most difficult things he had ever done, but he did it to protect Elizabeth like Sam had done.

Sam's murder is shown in a flashback as one of the people murdered by the man known as Raymond Reddington to hide his secret.
